Description
IMPELLA CIRCULATORY SUPPORT COMPONENTS FOR PORTLAND VA MEDICAL CENTER PORTLAND, OREGON. THE PURPOSE OF THIS MODIFICATION IS TO CORRECT THE QUANTITIES OF THE 2 LINE ITEMS IN THIS CONTRACT. THE ORIGINAL 2237 PROVIDED BY THE STATION INDICATED THAT THE QUANTITY FOR LINE ITEM 1 WAS 8 EACH, AND THAT THE QUANTITY FOR LINE ITEM 2 WAS 2 EACH. AFTER CONTRACT AWARD, THE STATION NOTIFIED CONTRACTING THAT THEY MADE AN ERROR THE QUANTITY FOR LINE ITEM 1 WAS SUPPOSED TO BE 2 EACH, AND THE QUANTITY FOR LINE ITEM 2 WAS SUPPOSED TO BE 8 EACH. THIS MODIFICATION WILL CORRECT THIS ERROR.
